Will see 16-18 patients per day.
The provider must be willing to work 3 months minimum with the option to extend. No exceptions.

Active DEA needed before starting
Credentialing takes 30-60 days (six weeks is standard for Locums who submit app./documents timely)
The following immunizations are required for credentialing (before start date): Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).
Expense Caps:
Airfare: Yes ($800/per flight or $1,600 round trip)
• Lodging: Yes ($2200/month cap)
• Rental: Yes ($1500/month cap)
Prior written approval is required if caps are exceeded
