SCHEDULE
• Coverage Type: Inpatient
• Schedule Type: Clinic and Call
• Call Type: Weeknight Beeper and 24 Hour Weekend Beeper Call
• Dates of Need: we need ongoing coverage about 1-2 weeks a month

• Provider Min # Shifts Required: 7-14 shifts a month
• Guaranteed Hours per Day: 8
• Hours Worked for OT to apply: 12 hours per day
• Rates to be Filled Out on Bid: Clinic and Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back.
OT Policy: Overtime must be pre-approved by medical director by end of shift.

Additional Details:
Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement):
• NOT reimbursed:
o Mileage from provider’s home to departing airport, as well as from airport to home upon return.
o Ride share (Lyft/Uber/etc.) and/or taxi from provider’s home to departing airport, as well as from airport to home upon return.
• WILL be reimbursed:
o Airport parking at provider’s departing airport if provider drives themselves to the airport.
o Insurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed.
o Ride share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from lodging to hospital/clinic, and return trips.
Please note, the providers all work together and share the shifts but if they are not willing to do night/weekends that would make it very hard for our perms so please ensure providers are willing to do so. Please note that providers will not be thrown onto night shifts until they have worked a few weeks on the day shift with other physicians. Once comfortable then they will be scheduled nights. Weekday Shifts can vary from 8-12 hrs. Please see new shift structure below and how they will be billed: OB Shift (7am- 7pm) -Provider will be inhouse from 7a-3p (billed at the regular hourly rate – 8hrs) -Then on call from 3p-7p (billed as one unit of the night call rate and if physically called in between 3p-7p then call back rate will apply immediately) Eve (2) Shift (3pm-7am) -Provider will be inhouse from 3p-11p (billed at the regular hourly rate- 8hrs) -Then on call from 11p-7a (billed as one unit of the night call rate and if physically called in between 11p-7a then call back rate will apply immediately).
